Enrollment Area
This network includes all Medical Associates Clinic providers and locations, and most of the family medicine providers in the Tri-state area. MercyOne Dubuque Medical Center and Unity Point Finley Hospital, along with hospitals within a 60-mile radius of Dubuque, are included in the network.
Tri-States Community Health Plan (CHP)
Enrollment Area
This network provides access to nearly all providers located in Dubuque and the Tri-state area. It includes all Medical Associates Clinic providers and locations, and all providers associated with Tri-State DOCS (Independent Physician Association). MercyOne Dubuque Medical Center and Unity Point Finley Hospital, along with hospitals within a 60-mile radius of Dubuque, are included in the network. The Community plan is available to employers who are domiciled in the five Iowa counties that the Tri-State Health Plans serves in addition to Jo Daviess County in Illinois.
Heartland Health Plan

Heartland Health Plan network includes a wide range of healthcare providers and facilities across Iowa, including MercyOne hospitals and affiliates surrounding Mason City, Cedar Falls, Waterloo, the Quad Cities, and Des Moines as well as Mercy Medical Center Cedar Rapids and affiliates. Members also have access to comprehensive healthcare through independent clinics like The Iowa Clinic, McFarland Clinic, Mason City Clinic, Physicians’ Clinic of Iowa (PCI) and also includes specialists throughout the region.
