IMPORTANT HEALTH COVERAGE TAX DOCUMENTS
Per federal law, health plans are not required to mail Form 1095-B for the reporting period. Therefore, you will not receive a Form 1095-B via postal mail. To obtain a copy of your Form 1095-B, contact Medical Associates Health Plans. Form 1095-B will be forwarded to you within 30 days of the date the request is received.
